[omniORB] omniORB4 omniNames and omniORB.cfg/sample.cfg

Hi Thomas,

 sample.cfg is a template for an omniORB4 config file, it
replaces the omniORB3 style config file. The ORBInitRef
parameter has been replaced by InitRef. The template is
full of very nice comments, it should be easy to adapt it
to your setting.
